I understand and agree that at Aumhaus, I am receiving bodywork, massage therapy, meditation, 1:1 soul nourishment, acupuncture, and vibrational sound healing therapies in a group or private session.

I understand and agree that bodywork, massage therapy, meditation, 1:1 soul nourishment, acupuncture, and vibrational sound healing and therapies are provided for stress reduction, relaxation, relief from muscular tension, and improvement of circulation and energy flow.

I understand and agree that bodywork, meditation, 1:1 soul nourishment, acupuncture, vibrational sound healing and therapies are not replacing or substituting for medical examination, medical diagnosis, or medical treatment.

I understand and agree that a licensed physician has verified my good health, physical and mental condition to participate.

I understand and agree that I will make the facilitator aware of any medical conditions I have prior to class.

I understand and agree that I am responsible in choosing to participate.

I understand and agree that any medical or mental health advice given is ultimately accepted by my own free will.

I understand and agree that Aumhaus is not claiming the ability to heal my body or mind.

I understand and agree that if I experience any pain or discomfort during any group or private session, I will immediately inform my facilitator so that adjustments can be made to my level of comfort. I will not hold the facilitator or Aumhaus responsible for any pain or discomfort I experience during or after my session.

I understand and agree that by participating in a group or private session, that there may be the possibility of detoxification symptoms or physical injury and I assume the risk and responsibility for any such results.

I understand and agree that at Aumhaus, there will be no sexual harassment allowed.

I understand and agree that massage therapy, bodywork, meditation, 1:1 soul nourishment, acupuncture, sound healing and therapies in private and group sessions are in entirely non-sexual in nature.

I understand and agree all services at Aumhaus are strictly professional and non-sexual. Any overt and suggestive statements, actions, and solicitation of prostitution will be cause for session to end immediately, full payment is due and authorities will be called. Aumhaus and their facilitators have the right to refuse service to anyone at anytime for any reason.

I understand and agree that Aumhaus is not responsible for any loss or damage of my personal belongings.

I understand and agree that I am responsible for the risks associated with traveling to and from Aumhaus.

I understand and agree that injuries may arise from my own or others' negligence and assume all related risks.

 

I understand and agree that photos maybe taken during the session. I am in the photos and give permission to Aumhaus to use for marketing.

I understand and agree that Aumhaus may make changes in their waiver agreement. It is my responsible to check back from time to time to see the updates.

I understand and agree that I have read the Policies Page at Aumhaus.com and abide by all the terms.

I acknowledge that I have read and completely understood Aumhaus' liability waiver and agree that it is a release of liability from the past, present and future relating to bodywork, massage therapy, meditation, 1:1 soul nourishment, acupuncture,  sound healing and therapies in a group or private session. By signing the agreement, I acknowledge that Aumhaus and their facilitators are not responsible for any and all claims or causes of action.

I understand and agree to waive and release, discharge, covenant not to sue and hold harmless to Aumhaus and their facilitators from any claims, liabilities, actions, damages, or costs of any kind arising out of this entire agreement. 

I understand and agree to not have anyone else acting on my behalf.

I understand and agree that my electronic signature below constitutes a real signature.
